The following guidance should be followed depending upon the current form of morphine being prescribed.

4 hourly morphine

Apply the patch and give the next 3 four hourly doses of morphine then stop regular morphine.

12 hourly morphine

Apply the patch at the same time as the last morphine dose then stop 12 hourly morphine.

24 hourly morphine

24 hourly morphine info

Apply the patch 12 hours after the last morphine dose then stop 24 hourly morphine.

CSCI morphine

Apply the patch continuing the infusion for a further 12 hours only.

If the patient is on a large dose of opioids, great care is required. Seek specialist palliative care advice.
